UCSC Miscellaneous Appeal Request Forms
Undergraduate Admissions occasionally receives appeals that do not fit in the categories described on the website, such as a missed deadline to accept a waitlist invitation or to submit a late application for admission.
Appeal Transmittal: A miscellaneous appeal must be submitted using this website. An appeal submitted by a person other than the applicant will not be considered.
Appeal Content: Provide a detailed explanation for the request and why your appeal should be considered. Include all pertinent information to ensure a comprehensive review can be provided.
Appeal Response: The decision regarding a student’s miscellaneous appeal will normally be completed within six weeks and communicated via email. In rare circumstances when additional information is required, resolution of the appeal review may take longer, and Undergraduate Admissions will inform the student of this within six weeks of the receipt of the appeal. If additional information is requested and not provided in a timely manner, the appeal will not be reviewed.
Appeal Deadline: An appeal to submit a late application must be submitted no later than three months prior to the start of the quarter.
Appeal Content: Provide a detailed explanation for the request and why your appeal should be considered with the following information.
1. Reason for missed deadline
2. Reason why late application request should be considered
3. Date of birth
4. City of permanent residence
5. Intended major
6. Email address
7. Mailing address
8. List of all courses currently in-progress or planned
9. The University of California application number (If a University of California application has already been submitted and UC Santa Cruz is to be added).
For first-year applicants, the appeal package must also include the following. If any of this mandatory academic information is missing, the appeal is not complete and will not be considered.
Self-reported test scores (SAT/ACT scores, if taken)
Self reported TOEFL/IELTS scores (if required)
Self reported AP/IB exam scores, if taken
High school transcript(s), unofficial copies are acceptable
College transcript(s) from all institutions where the applicant was registered at any time, whether or not courses were completed, unofficial copies are acceptable
For transfer applicants, the appeal must also include the following. If any of this mandatory academic information is missing, the appeal is not complete and will not be considered.

Appeal Deadline: An appeal to accept a waitlist offer must be submitted no later than May 1 for first year students and no later than June 1 for transfer students.
Appeal Content: Please provide a detailed explanation for the request and why your appeal should be considered. Include all pertinent information to ensure a comprehensive review can be provided.
Appeal Deadline: An appeal to submit a late SIR must be submitted no later than 3 months prior to the start of the quarter.

High school transcript(s) with final grades, unofficial copies are acceptable (First-year only)
College transcript(s) with final grades from all institutions where the applicant was registered at any time, whether or not courses were completed, unofficial copies are acceptable
Appeal Deadline: Deferral requests must be submitted no later than 30 days prior to the start of the quarter and will only be considered from students who have already submitted a Statement of Intent to Register (SIR) to UC Santa Cruz and have met all the conditions of admission. Students unable to SIR are encouraged to apply for the next application cycle.
Appeal Content: Provide a detailed explanation for the request and why your appeal should be considered with the following information.
