UCSC Schedule Change and/or Grade Issue Request Form
If you have a schedule change and/or a grade issue to report please fill out this form to provide a complete update of your coursework for this academic year. The Office of Undergraduate Admissions will respond to your submission within 2-3 weeks via email.
Please only submit one form even if there are multiple changes.
A Student ID is required to submit a Schedule Change and/or Grade Issue Request form.
